Website for MedsCheck

MedsCheck is a free program offered by the Ontario Ministry of Health for people who are taking 3 or more medications for chronic conditions. The web page provides information about MedsCheck programs for people with diabetes who are living at home or in long-term care homes. A fact sheet "Medication Shouldn't be Confusing" is available in various languages and the Ontario drug programs information can be accessed from this page.

You need to bring some information and items to your MedsCheck appointment. Click on this link for information on how to prepare for your appointment: http://health.gov.on.ca/en/public/programs/drugs/medscheck/your_appointment.aspx
